ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) who demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for a panel of up to 450 patients. The PCP acts as a health coach, helping patients set and achieve health goals, and leads a care team including medical assistants and coordinators. Responsibilities include providing care for acute and chronic illnesses in older adults, managing patient care and outcomes, collaborating with hospitalists and specialists, and participating in quality and risk management programs. The PCP must be available and accessible to patients, provide high-quality care, and continuously improve leadership and clinical skills. Education requirements include an MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty, with board eligibility or certification, a current DEA number, and BLS certification within 90 days of employment.
