ChenMed, a physician-led and mission-driven primary care organization, is seeking a Nurse Practitioner to join their team. The Nurse Practitioner will be responsible for providing care for patients with acute and chronic illnesses, primarily older adults, managing a patient panel of up to 450 patients. Responsibilities include assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. The role involves working independently and collaboratively with other healthcare professionals, engaging with hospitalists, and leading a care team. The Nurse Practitioner will also participate in risk and quality management programs, clinical meetings, and help maintain a positive culture within the center. The position requires accountability for patient outcomes, coaching patients for health, simplifying care actions, and meeting performance metrics. Candidates must have an ARNP or similar advanced nursing degree, current Nurse Practitioner certification in the state of practice, and preferably board certification by AANP or ANCC. A minimum of 1 year clinical experience is preferred but new graduates will be considered. Basic Life Support certification is required within 90 days of employment. The role may require local travel and occasional regional or national travel for training and recruiting. Proficiency in Microsoft Office and fluency in English are required.
